You've seen alot of colorful stuff in Nintendo games here and now but back then it was something different when you were a child, I gotta say, my first scary moment was not the N64 with Super Mario 64's Piano and Ocarina of Time's Well or Shadow Temple moments but the intro to Earthbound is the scariest paranoid thing Ive ever witnessed when i was a kid. I mean when the first thing you see in the intro was UFOs attacking the earth with a disturbing track growing louder while fading to white, how can you not panic at that age.
What was your scariest moment in a Nintendo game back then?

Haha, the piano got me too. If anything, the thing that freaked me out the most was the alien invasion in Zelda: Majora's Mask. The sole fact that there was aliens freaked me out. I failed the first time, then saw Romani abducted and left with no memory the day after...
The nightmares.

I was playing a friend's copy of Ocarina of time, when, at the time, I knew nothing about videogames....
I got the last of the 3 stones, thrust the master sword into the pedestal, left the temple of time...
ReDeads everywhere.
I stopped playing, and didn't return to Zelda until Twilight Princess back in 06....

Oh my god how could i forget Majora's Mask! Ugh that game was a stuff of nightmares with epic Zelda gameplay with it. Even the mirror shield got a disturbing design too.

Probably the original Luigi's Mansion.

The deadhands were worse. You know those bloody monsters in the Shadow Temple that tried to eat you.

Agreed. Ikana Valley pretty much sums up half of my freaky first-time experiences with that game.

Since I never had a Nintendo childhood, I wasn't really scared of anything till Resident Evil on PSX. However some moments with Jurassic Park for Master System were pretty adrenaline pumping.

Ocarina of times ReDeads and the graveyard when you're a adult.
Twilight Princess with the scary cutscene, the one where theres like 4 dark links and 1000 of that girl upside down laughing... oh god
